A. The SLUMS Examination
A screening instrument for the detection of mild cognitive impairment and dementia.

B. The Major Depression Symptoms Checklist
Developed by Lyndon D. Waugh M.D., use of this checklist is a great way to master the multi-dimensions of depression. This lists symptoms in "Domains" and has an additional History section which is a reminder that it's important to take a history specific to the presenting problem apart from other information gathered.

C. ADHD Self Report
Developed by Lyndon D. Waugh M.D., this instrument is most appropriate for ages 16 and older and not only paints a picture of the many ways a person can experience ADHA, but uses descriptions that avoid clinical lingo.

D. Understanding, Assessing and Treating Anxiety Problems in Children and Adolescents
